Wellbeing Navigators

This service is to help you maintain or improve your health & wellbeing, and engage & participate in your local community. Wellbeing Navigators will support you to connect with other carers locally, identify and navigate local health and wellbeing services and help you find groups / activities that you'll enjoy.

Carers can select up to three options from the Wellbeing Navigator Service:

Health and Wellbeing check: an initial assessment of health and wellbeing to identify areas of personal concern, medical need and/or refer to relevant services 

Activity Support: i) Support to attend local groups or physical activities which will maintain/improve health and wellbeing  for up to 3 sessions; OR ii) One to one session with a Navigator to focus on physical activity for up to 3 sessions.

Passport to Wellbeing: A six-week peer support group to identify how building small actions into day to day life can make a big difference to levels of wellbeing. Importantly you'll have the opportunity to meet and chat with other ‘local’ carers and build a local resource of support.
